National Security Agency experts in the US are looking into why three ballistic missile crew members in North Dakota fell asleep this month, while in charge of nuclear devices.
The crew members held the vital codes which can launch nuclear rockets from the US.
Officials say the three officers were behind locked doors and had authorisation to communicate with the missiles through launch codes.
While it has been found that the missile launch codes remained secure, the July incident on July 12th has once again put the military services under scrutiny.
Last month, Defence Secretary Robert Gates put a broom under the Air Force leadership, telling them they had failed to deal properly with a series of nuclear-related mishaps.
Gates sacked the Air Force secretary and chief of staff after a scathing report on the mistaken shipment to Taiwan of four Air Force fusing devices for ballistic missile nuclear warheads.
They were also punished for an incident last year in which a B-52 bomber was mistakenly armed with six nuclear warheads when it flew from an air force base near the Canadian border to Barksdale Air Force Base in the southern state of Louisiana.

What an exaggeration!
This article is just another example of a reporter writing about a topic that he knows absolutely nothing about and taking the liberty to embellish and exaggerate the facts for the sake of sesationalism.
The officers involved did not have “authorisation” to use these codes, nor where they locked behind closed doors. They were part of a team sent out to change codes and were guarding the device used to insert them into the system. They were not inside the launch control center but in an above ground support building.
While falling asleep in this situation is a serious lapse of duty, there are many other layers of protection built in to our nuclear system to protec it from this type of procedural violation.
I’ll conclude this with my favorite quote: Don’t get stuck on stupid...reporters” General Russel Honore' 1996.

Human error is the belief I have for both incidents!
The delivery of nuclear materials to Taiwan bothers me and makes me wonder about payoffs and Clintonesque escapades with China!
Had the delivery been somewhere in the middle east I would be even more concerned!
The incident in question this news piece is nothing more then a boring watch in the middle of no where!
Since you have never served in the military you have no concept of the actual tedious repetative nature!

WASHINGTON July 29, 2008 - Report says US national security system is outdated and needs massive changes

In a report aimed at the next president, security specialists are proposing a vast overhaul of the U.S. security system, declaring it problem-plagued. The report, a copy of which was obtained by The Associated Press, said frequent feuding and jurisdictional disputes among Cabinet secretaries and other agency heads force the president to spend too much time settling internal fights. Time and money are wasted on duplicative and inefficient actions, slowing down government responses to crises, the report said.
The president and his top advisers focus on day-to-day crisis management rather than long-term planning, “allowing problems to escape presidential attention until they worsen and reach the crisis level," said the report, to be issued later in the week. The study, mandated by Congress, was undertaken by the Project on National Security Reform. The research was conducted by more than 300 national security experts from think tanks, universities, federal agencies, law firms and corporations. A final version with recommended reforms, including a proposal for new security legislation, is expected to be issued in October.
“We will approach whoever is elected before the inauguration and will be having discussions with campaign staffs between now and the election," James R. Locher III, executive director of the project, said in an interview. “We will draft presidential directives that can be imposed by the new president immediately," Locher said. Thomas J. Pickering, a former career diplomat who served as U.S. ambassador to the United Nations, said in a statement that the findings would be valuable to the next president and to Congress. “Our national security system is broken and needs fixing," he said.

Government officials have been quietly stepping up counterterror efforts out of a growing concern that al Qaeda or similar organizations might try to capitalize on the spate of extremely high-profile events in the coming months, sources tell ABC News. Security experts point to next month’s Olympics as evidence that high-profile events attract threats of terrorism, like the one issued this past weekend by a Chinese Muslim minority group that warned of its intent to attack the Games. This is what the Department of Homeland Security is quietly declaring a Period of Heightened Alert, or POHA, a time frame when terrorists may have more incentive to attack. According to drafts of government memos described to ABC News, the period would run roughly from this August through July 2009. During this time, homeland security analysts will be asked to redouble efforts to study terrorism leads. And a number of agencies will be asked to review emergency response plans to a variety of attacks, from improvised explosive devices (IEDs) to biological weapons.
Officials also are being asked to make sure they are prepared for all contingencies during the transition from the Bush administration to that of the next president. In a recent interview, FBI director Robert Mueller told ABC News of his concerns for homeland security. “When you have a series of events like this which are very public, where you have a number of people that are congregated together, we take additional precautions," he said.
